The stroke resulted in
homonymous hemianopsia, or hemianopia, the lack of a column of vision in each eye.
Blurred vision,
homonymous hemianopsia as well as cortical blindness in some cases are common manifestations of the visual disturbance.
On neurological examination, right sequel
homonymous hemianopsia and frust right hemiparesis were found.
The stroke team found the patient awake and alert but with a decreased gag reflex, dysarthric with acute left hemiparesis with no sensation in the left upper or lower extremities, and a left
homonymous hemianopsia field cut.
Examination showed blood pressure of 115/80 mmHg, regular heart rate of 90 per minute, Glasgow coma scale (GCS) of 12, expressive aphasia, left facial nerve palsy of upper motor neuron type with dense left sided hemiplegia and left
homonymous hemianopsia (NIH Stroke Scale score 17).
Her face was symmetric, and cranial nerve examination demonstrated a right
homonymous hemianopsia.
Neurological examination revealed higher normal mental functions, decreased visual acuity in both eyes, left
homonymous hemianopsia, bilateral pale discs with normal pupillary reflexes and extraocular movements.
He has poorly controlled type II diabetes mellitus; obesity (body mass index 40 kg/ [m.sup.2] ); complicated cryptogenic cirrhosis with prior esophageal varices, portal gastropathy, splenomegaly, and no encephalopathy; surgically treated colon adenocarcinoma; and bilateral thalamic and right occipital infarcts with residual left
homonymous hemianopsia and vertical gaze paresis.
