Symptomatic excyclotorsion following inferior transposition of both medial rectus muscles in patients with bilateral
trochlear nerve palsy.
In classical
trochlear nerve palsy, the affected eye adopts a hypertropic and out-rotated position in primary gaze leading to complaint of vertical and torsional diplopia, especially when attempting to look down and in (eg when reading or walking down stairs).

All animals were prepared for aseptic surgery and given local analgesia by giving nerve block to cornual branch of lachrymal nerve in sheep and cornual branches of lacrimal and infra
trochlear nerves in caprines with 2 ml of 2% Lignocaine hydrochloride at each site subcutaneously.
The anomaly of the PCP may also pose an additional obstacle while performing clinoidectomy operations and there are chances of inadvertent injury to the internal carotid artery, the oculomotor and the
trochlear nerves.
