Large effusion (> 50% of hemithoracic volume) was seen in 70.91% cases (Figure 1),
hydropneumothorax was seen in 9 cases (16.36%) (Figure 2) and small encysted pleural effusion in 12.73% patients.
Chest X-ray was suggestive of left
hydropneumothorax and HRCT thorax revealed mediastinitis with left sided empyema (Figures 1 and 2); intercostal drainage tube was inserted, which revealed frank pus.
Hydropneumothorax (Definition) concurrent presence of air and fluid in the pleural space.
In our study, 23 cases of pleural lesions were noted of which 18(78%) had pleural effusions, 2(9%) had
hydropneumothorax and 3(13%) had empyema.
Five cases in combined diagnosis developed
Hydropneumothorax and all cases were recovered by intercostal tube drainage and O2 inhalation.

When rupture of the hydatid cyst occurs into the pleural space, complications like
hydropneumothorax and empyema may occur.
Even though Boerhaave's syndrome typically manifests as mediastinitis and sepsis, there are reported cases of pneumothorax (5),
hydropneumothorax (6), heamothorax (7,8) secondary to esophageal rupture.
One (1.67%) case showed pneumothorax,2(2.33%)cases showed
hydropneumothorax, 2(2.33%)cases showed "air crescent" sign.
fumigatus isolate was collected by bronchoalveolar lavage from a woman with chronic obstructive pulmonary disease, interstitial pulmonary fibrosis, and hypersensitivity pneumonitis, while she was hospitalized for
hydropneumothorax and bacterial pneumonia secondary to trauma; no antifungal treatment was given.
Pneumothorax,
hydropneumothorax, empyema, and bronchopleural fistula are other uncommon RA pleural complications, and the treatment can be challenging [3].
The CXR showed a large right-sided
hydropneumothorax, and the esophagus was air-filled down to the distal segment (Figure 1).
