The current study was planned to share our experience with the xiphisternal approach with successful implantation of both atrial and ventricular leads.
The retrospective study was conducted at Aga Khan University Hospital, Karachi, and comprised data of patients aged up to 16 years who underwent epicardial dual chamber permanent pacemaker insertion via xiphisternal incision between April 2011 and August 2016.
The pacemaker implantation via xiphisternal incision was successful in all 10(100%) patients.
In this study, lead placement via xiphisternal approach was compared with lateral thoracotomy and median sternotomy.
The point was located on the upper abdomen, on the line between the xiphisternal junction and pubic symphysis (a total of 13 equal parts), 4 equal parts inferior to the xiphisternal junction.
For anteroposterior measurement, the front tip of the caliper was placed on the xiphisternal junction.
The fourth intercostal space was used in circumferential measurement by tape measure and the xiphisternal junction in anterior to posterior measurement using calipers.
The gall bladder was then dissected from its bed and extracted from either the umbilical or the sub xiphisternal port.
As hyperinflation develops the anterior-posterior diameter of the chest and the xiphisternal angle increases.
The inner measuring electrodes were placed around the base of the neck and around the thorax at the level of the xiphisternal joint.
