If the bedside echocardiogram was able to detect the aortic
valve vegetation, the patient would have been evaluated directly by TEE rather than TTE followed by TEE.
The diagnosis of bacterial endocarditis with severe aortic
valve vegetation was made after a transthoracic echocardiography was performed.

A resident who came to do a physical exam asked, "Has anyone told you that you had a heart murmur?" An echocardiogram revealed a mitral
valve vegetation, which had gradually destroyed the valve.
TTE done at week 2 of treatment and at the end of antimicrobial therapy showed complete disappearance of the mitral
valve vegetation.
A transesophageal echocardiogram was performed to better visualize the mitral
valve vegetation, revealing a 4.3 cm elongated mass attached to the base of the anterior mitral valve leaflet (Figure 3), accompanied by severe mitral regurgitation.
A preoperative transesophageal echocardiogram (FIGURE 3) confirms earlier findings and additionally reveals a small mitral
valve vegetation. At surgery, the aortic valve is resected and replaced; a perivalvular abscess is drained.
