thorn sign Imaging A vaguely defined spicular radiologic shadow that tapers medially from the lateral chest wall, which corresponds to a thickening of the minor fissure, most commonly seen in a right-sided pleural effusion; since the finding appears in most positions, the TS should prompt a lateral decubitus film and ultrasonography to confirm the presence of pleural fluid.
