shared care agreement

An agreement between a hospital consultant and a GP in the UK regarding the care of specific patients, especially as relates to the cost and administration of certain drugs. In a shared care agreement, the patient is referred to the consultant, who writes a treatment plan which, assuming the drug does not have unpredictable side effects or require close monitoring, the patient’s GP may agree to shoulder on a case-by-case basis.
