paraglottic space

paraglottic space

the space on each side of the glottis bounded laterally by the perichondrium of the thyroid cartilage and the cricothyroid membrane and posteriorly by the mucous membrane of the pyriform sinus; anterosuperiorly it extends into the preepiglottic space. It is an important route of transglottic and extralaryngeal spread of carcinoma of the larynx.

The paraglottic space is seen better with a lateral thyrotomy technique.
The paraglottic space lies anterolateral and deep to the ventricles and saccules, and the pre-epiglottic space is anterior to the base of the epiglottis.
Embryologically, the supraglottis has a separate origin from the glottis, explaining why supraglottic tumors typically do not traverse the ventricle to reach the glottis unless the paraglottic space is involved.
Hence, the migration of the foreign body into the paraglottic space is a rarer event.
We advocate the V-shaped lateral thyrotomy as a single approach for resection of the combined laryngocele as it provides wide accessibility, good exposure, and safe laryngocele dissection in the paraglottic space and ensures resection of the entire lesion under direct visualization.

Postcontrast computed tomography (CT) scan of the neck showed a well-defined intensely enhancing mass arising from the right paraglottic space and bulging into the right pyriform sinus (Figure 2).

Augmentation of the interarytenoid space is a simple and innovative alternative to the traditional paraglottic space injection.
An insufficient view of the surgical site, anterior commissure involvement in the craniocaudal direction (T2), invasion of the posterior paraglottic space with arytenoid fixation, massive infiltration of the pre-epiglottic space, and the presence of minor thyroid cartilage erosion (T3) are the most controversial situations with regard to the use of TLM in glottic and supraglottic cancers (24).
The edema was located at the aryepiglottic fold; it encompassed the arytenoid cartilage and false vocal fold, and it extended into the paraglottic space. The patient was treated with an oral antibiotic and a steroid.
Contrast-enhanced computed tomography (CT) of the neck demonstrated a homogeneous soft-tissue lesion that involved the left paraglottic space and glottis and extended into the subglottis (figure 1).
