lacrimal

pertaining to tears.
lacrimal apparatus a group of organs concerned with the production and drainage of tears; it is a protective device that helps keep the eye moist and free of dust and other irritating particles. The lacrimal gland, which secretes tears, lies over the upper, outer corner of the eye; its excretory ducts branch downward toward the eyeball. A constant stream of tears washes down over the front of the eye and is drained off through two small openings located in the inner corner of the eye. Through these openings the tears pass into the lacrimal canaliculus, then through the lacrimal sac into the nasolacrimal duct and finally into the nasal cavity.

In comparison of both, endonasal DCR has certainly many advantages; minimally invasive procedure, better aesthetic results with no cutaneous scar, preserves lacrimal pump system, faster resumption to daily activities, any intranasal pathology that might have caused obstruction can be addressed at the same time, active infection of the lacrimal system is not a contraindication to the surgery, it is much less bloody than the external approach, the per-operative time is shorter.
However, the procedure leaves a cutaneous scar and the potential for injury to medial canthal structures, cerebrospinal fluid rhinorrhea, and functional interference with the physiological action of the lacrimal pump (14).
The advantage of an endoscopic approach over external dacryocystorhinostomy is the lack of scar, shorter operating time, done as an outdoor procedure under local anesthesia and less chances of interfering with the physiological lacrimal pump mechanism.
