CT-angiography, advanced hemodynamic monitoring, and determination of catecholamines,
chromogranin A in plasma, and 5-hydroxyindoleacetic acid (5-HIAA) in urine were performed.
(c) Immunohistochemical stain positive for
chromogranin and (d) negative for PSA.
Estimated
chromogranin A concentration was in the normal range (36, 3ng/ml).
Additionally, plasma
chromogranin A was markedly raised (16-fold) at time of consultation and prior to surgery (15-fold).
[6] One case of NK/T-cell lymphoma nasal type was diagnosed which showed reactivity for LCA, CD3, vimentin and non-reactive for CD5, EMA, NSE and
chromogranin. However further IHC study for CD56 and granzyme B was advised for confirmation.
Immunohistochemically, the tumour cells were negative for PSA and positive for
chromogranins (Fig.
Without a clinical history the diagnosis of these tumors can be very challenging, yet an immunohistochemistry panel consisting of melan-A, alpha inhibin, D11, calretinin, NSE, synaptophysin and
chromogranin can be helpful in establishing the diagnosis.
CD 117, DOG 1, S100, CD34, desmin, and neuroendocrine markers such as
chromogranin, synaptophysin, neuron-specific enolase, CD56, CD57, and PGP 9.5 are negative (1, 5).
Low Power View of H and E Carcinoid Tumour with
Chromogranin (IHC) Positive Stain
With immunohistochemical staining the tumour cells showed strong diffuse positivity for neuron-specific enolase (NSE),
chromogranin, and synaptophysin (Figure 5, 6, 7) and were negative for cytokeratin, TTF-1 & LCA.
One of the elements of glucagonoma diagnosis is histopathological examination including immunohistochemical staining such as
chromogranin or synaptophysin specific for neuroendocrine tumors.
