A standardised multidisciplinary format for providing mental health care, in which the service user and, where appropriate, his/her carer, are involved in developing an appropriate package of care.

Care programme approach elements
• An assessment of the patient's health and social care needs.
• A written care plan to meet the assessed needs. The patient is involved in drawing up the care plan.
• Regular reviews of the patient's care plan.
• A named mental health worker (care coordinator) who is responsible for the patient care under the care programme approach.
