Walnut Brain

A term referring to a brain with narrowed gyri and widened sulci which is seen in extreme frontal and temporal lobe atrophy, with relative sparing of the posterior brain. The finding is characteristic of Pick’s disease, and has been fancifully likened to the appearance of a walnut; as atrophy evolves, compensatory ventricular enlargement—ex vacuo hydrocephalus—occurs
