A rare—1:35,000 pregnancies—complication of monochorionic twin pregnancy, involving an acardiac twin with structural defects incompatible with life, and an otherwise normal 'pump' twin. The ‘plumbing defects’ can cause TRAP-twin reversed arterial perfusion syndrome and high output heart failure in the pump twin
