In this pilot study, we have evaluated the correlations between mCdPT, mCaPT, MVD, and EA in a series of 31 PDACP having undergone surgery and our results suggest an association between
tryptase and microvascular bed.
The key mediator is
tryptase. In addition to environmental exposure such as Hymenoptera stings, Viper venom and poison ivy, other causes include several drugs and number of conditions (angioedema, bronchial asthma, food allergy, serum sickness).
Measured
tryptase levels following these reactions have always been normal.
Many preformed mediators are involved in an anaphylactic reaction, including histamine,
tryptase, chymase, mast cell carboxypeptidases, platelet activating factor and others.
Tryptase levels an indicator of mast cell activation in systemic anaphylaxis and mastocytosis.
The measurements of serum specific IgE directed to the [beta]-lactams, serum chymase and
tryptase levels were not performed (due to a lack of laboratory support).
Although the use of mast cell
tryptase, skin testing, and in some circumstances radioimmunoassay are now standard in the investigation of such patients, there is no doubt that the best test to determine the safety of subsequent anaesthesia is an uneventful anaesthetic.
Once the person has been sensitized and has had time to produce IgE, the next administration of penicillin may activate the primed mast cells and basophils, and lead to the systemic release of histamine and other mediators (e.g., mast cell
tryptase, leukotrienes--see Figure 1).
Tests include skin-prick testing, blood tests, multi-allergen IgE antibody screening assays, mast cell
tryptase and CAST testing.
Assays for
tryptase have proved useful in the diagnosis of anaphylaxis, but increased circulating levels of this mast cell product often cannot be detected.
