The thyroid and the parathryoid glands are wrapped in a part of the pretracheal fascia, which is deficient on the posterior aspect of the isthmus and most of the posteromedial surface of the lobe and so extends laterally to blend with the carotid sheath and the prevertebral fascia.
The posterior surface of the isthmus is firmly adherent to 2, 3 and 4th tracheal ring and pretracheal fascia is here fixed between them.
An elliptical incision of the entire defect was carried out down to the pretracheal fascia. The dissection followed a fibrous cord underlying the sinus.
