Practitioners' Reporting Network, USP,
Three separate programs designed to collect practitioners' experience with unreliable drug products, defective medical devices, drug problems with radiopharmaceuticals, and medication errors. Practitioners and pharmacists report their experience to the United States Pharmacopeia, 12601 Twinbrook Parkway, Rockville, MD 20852. The USP receives the reports and publishes the results. Drug problem reports and medical device and laboratory product problem reports may be made by calling (800) 638-6725; medication errors may be reported by calling (800) 23ERROR.
