The Chronic Disease Score is a comorbidity measure obtained from a weighted sum of the use of medications from 30 different classes (Von
Korff, Wagner, and Saunders 1992).
The Community Activation Survey involved a cohort of representatives of key community organizations identified through reputational sampling methods (Wickizer, Von
Korff, Cheadle, et al.
Williams, Jr, MD, MHS; Kathryn Rost, PhD; James Barrett, MD; and Michael Von
Korff, ScD.
Collaborative Care enriches the depth of the encounter, and (anecdotally) contributes positively to patient satisfaction (Bray & Rogers, 1997; Von
Korff & Myers, 1987).

Korff and coworkers (26) of continued pain and functional disability at follow-ups of 90 days and 1 year, respectively, are very similar to our own.
While in some settings the disease management program is a coordinated program between a primary care practice and a separate mental health agency, the studies on which the effort is modeled (Katon, 1995; Katon, von
Korff, Lin, et al., 1995) were fully integrated and the overall effort is in the direction of integration.

Not only do patients frequently want their primary-care physician to treat their problem, even though they are aware that it is depression, anxiety, marital, or family problems (Bray & Rogers, 1997), but also 50-70% of patients with mental health problems are seen exclusively in primary-care settings with no referral elsewhere (Katon, Von
Korff, Lin, et al., 1995; Marrow, Regier, Rae, et al., 1993).
Population focused care of chronic illness requires several organization-wide interventions including provider training, patient education, promotion of self-management, decision support from experts, evidenced-based approaches, gaining community support, and high availability of clinical information (Katon, Von
Korff, Lin, et al., 1997; Wagner, Austin, & Von
Korff, 1996).
Structural changes in integrating mental health professionals into primary care delivery (Katon, Robinson, Von
Korff et al., 1996).
