On immunohistochemical study, keratin 7 was diffuse positive with chromogranin and SYNP, and keratin 20 was negative.
These features were suspicious for a high-grade subtype of urothelial carcinoma, and thus immunohistochemistry was performed with a Ventana Benchmark Ultra using antibodies to bcl-2 (mouse monoclonal, ready-to-use, Ventana), keratin 20 (rabbit monoclonal, ready-to-use, Ventana), desmin (mouse monoclonal, ready-to-use, Ventana), p16 (mouse monoclonal, ready-to-use, Ventana), p40 (mouse monoclonal, ready-to-use, Ventana), GATA3 (mouse monoclonal, ready-to-use, Cell Marque), p53 (mouse monoclonal, ready-to-use, Ventana), and OSCAR pankeratin (mouse monoclonal, ready-to-use, Cell Marque).
Immunohistochemistry showed tumor positivity for vimentin, smooth-muscle actin and synaptophysin and negativity for the keratin 7, keratin AE1/AE3, keratin 20, keratin 5/6, chromogranin, KIT (CD117), CD34, CDX2, CD56, protein-S100, p63 and desmin antigens (Figures 5 and 6).

Not shown: negative keratin 20, TTF-1, napsin A, and CDX2.
Once they had defined these groups, they were able to pinpoint 2 genes that would accurately define them [[KRT20.sup.+]/[top-crypt.sup.high] (group 1), [KRT20.sup.+]/ [top-crypt.sup.low](group 2), and [KRT20.sup.2]/[top-crypt.sup.low] (group 3), where [KRT20.sup.2] is the keratin 20 gene and top-crypt is any one of the following genes: CA1 (carbonic anhydrase I), MS4A12 (membrane-spanning 4-domains, subfamily A, member 12), CD177 (CD177 molecule), and SLC26A3 (solute carrier family 26, member 3)].
