islet transplantation

islet transplantation

An alternative to pancreatic transplantation in patients whose conventional diabetic control is threateningly poor and whose risk of diabetic complications and shortening of life is considered high. A lesser degree of immunosuppression is required than panceras transplantation and the surgery is minor. Purified ISLETS OF LANGERHANS are digested from a donor pancreas and are infused through a catheter passed through the liver into the portal vein from whence the islets pass into the sinusoids of the liver. The method is still considered experimental.

Although islet transplantation is considered to be effective in those cases, a new therapy is desired due to the issue of shortage of donors.
To date, pancreatic islet transplantation is an emerging therapeutic option for diabetic patients (4).
Still, 20 years of research and clinical trials hasn't produced an effective islet transplantation therapy because multiple donors are needed, the current method of delivering islets through the portal vein of the fiver is too invasive and the human immune system tends to destroy a large percentage of transplanted islets.
Foster, Ph.D., from the University of Iowa in Iowa City, and colleagues evaluated HRQOL, functional health status, and health utility before and after pancreatic islet transplantation in CIT Consortium Protocol 07 trial participants.
Previously reported clinical outcomes from the trial showed that islet transplantation prevents severe hypoglycemia and improves blood glucose awareness and control.
All recipients were 10-week-old male rats weighing 250-300 g at the time of islet transplantation. Two or three rats were used as islet donors for one recipient.
In the vast majority of islet transplantation and cultivation approaches, alginate has been used as the encapsulation material [58].
We chose the BETA-2 score [22], which integrates biomarkers extracted from a morning fasted blood sample (i.e., C-peptide [nM], plasma glucose [mM], [HbA.sub.1C] [%]) and DIR (total units/kg body weight) and which was validated in the setting of islet transplantation in patients with T1D.
Islet replacement therapy (insulin-secreting pancreas transplantation or islet transplantation) enables the physiological regulation of blood glucose and precise maintenance of glycemia, which is not attainable by other modern interventions, including insulin pumps and/or continuous blood glucose monitoring therapies.
