Central sensitization, neuroplasticity Anesthesiology A physiologic ↑ sensitization of excitable neurons, coupled to a ↓ threshold for peripheral afferent pain terminals; windup may be mediated by excitatory amino acid neurotransmitters and neuropeptides; NMDA receptor inhibitors prevent 'windup' and may play a central role in managing chronic pain.
