visiting nurse,
a nurse who is responsible for a group of patients in a home setting, usually in a defined geographic area. The nurse makes visits to provide skilled nursing care as prescribed by a physician, particularly for persons unable to leave home for professional care, and to educate patients in matters of self-care.
