uncal herniation

uncal herniation
[ung′kəl]
Etymology: L, uncus, hook, hernia, rupture
a condition in which the medial part of the temporal lobe protrudes over the tentorial edge as a result of increased intracranial pressure. If uncorrected, the progressive disorder causes pressure on the brainstem after first impinging on the third cranial nerve. A dilated pupil on the side of the herniation is a diagnostic sign of the disorder.

caudal transtentorial herniation transtentorial herniation.
central herniation a downward shift of the brainstem and the diencephalon due to a supratentorial lesion, causing Cheyne-Stokes respirations with pinpoint nonreactive pupils.
cingulate herniation a shift of the cingulate gyrus to below the falx cerebri.
disk herniation (herniation of intervertebral disk) (herniation of nucleus pulposus) herniated disk.
tentorial herniation transtentorial herniation.
tonsillar herniation protrusion of the cerebellar tonsils through the foramen magnum.
transtentorial herniation downward displacement of medial brain structures through the tentorial notch by a supratentorial mass, exerting pressure on the underlying structures, including the brainstem; this is a life-threatening situation because of pressure on the third cranial nerve, with symptoms including dilated, nonreactive pupils, ptosis, and a decreased level of consciousness. Called also caudal transtentorial herniation, tentorial herniation, and uncal herniation.

nucleus pulposus herniation
rupture or prolapse of the nucleus pulposus into the spinal canal, or against the spinal cord. See also intervertebral disk disease.
transtentorial herniation
downward displacement (caudal transtentorial herniation; uncal herniation) of the medial brain structures through the tentorial notch by a supratentorial mass, exerting pressure on the underlying structures, including the brainstem.

Brain herniation, herniation syndrome, transtentorial herniation Neurology A condition in which the uncus–the anterior hooked end of the parahippocampal gyrus on the basomedial surface of the temporal lobe of the brain is displaced due to ↑ intracranial causing progressive brain and brainstem damage Etiology Cerebral edema due to head injury, space-occupying lesions–primary and metastatic brain tumors or other lesions within the skull, bacterial meningitis; UH compresses cranial nerve III, the midbrain, and the posterior cerebral artery, leading to coma and respiratory arrest. Cf Cerebellar herniation.

However, the patient exhibited no other signs of increased intracranial pressure or uncal herniation and had no decline in mental status.
Examination of the brain showed obstructive hydrocephalus, bilateral uncal herniation, flattening of the cerebral gyri, and an intact cysticercus compressing the inferior 4th ventricle (Figure 1).
The MRI was repeated and revealed complete effacement of the right frontal horn, with increasing vasogenic edema extending into right midbrain, significant brainstem compromise, and impending risk of uncal herniation.
