A data collection form used in pathology departments in the UK for audit, documentation, staging, etc. The proformas with the greatest impact on interdepartmental communication of critical patient management data are the minimum data sets produced or sanctioned by the Royal College of Pathologists for cancers of specific body sites. The cancer proformas are used by oncologists for staging, by surgeons to assess adequacy of resection, and by radiologists to confirm the diagnosis of presumed cancer.
