Microscopic hematuria with dysmorphic red blood cells, red blood cell casts and non-nephrotic range proteinuria are clues to renal involvement which may be initially asymptomatic, but can progress rapidly to renal failure.
14) Garel et al (15) reported two cases of mesangial IgM glomerulonephritis associated with hepatitis A, and Ilan and Galun (16) reported a patient with acute hepatitis A infection and presumed glomerulonephritis based on proteinuria, red blood cell casts, and microscopic hematuria, in the absence of any other etiologic agent(s).
This case demonstrates an unusual presentation of Wegener's granulomatosis, typically characterized by the recognized triad of upper and lower respiratory symptoms, such as epistaxis and dyspnea, and renal disease manifesting as proteinuria, hematuria, or red blood cell casts.
