A health and social care team in the UK for clients who cannot be effectively managed in usual primary care settings. PCLTs have a central role in organising and delivering services and work closely with GPs, statutory and non-statutory agencies, delivering patients between services as needed. The team offers risk assessment of clients, advice and short- to medium-term psychological therapies.
