paraglottic space

paraglottic space

the space on each side of the glottis bounded laterally by the perichondrium of the thyroid cartilage and the cricothyroid membrane and posteriorly by the mucous membrane of the pyriform sinus; anterosuperiorly it extends into the preepiglottic space. It is an important route of transglottic and extralaryngeal spread of carcinoma of the larynx.

The edema was located at the aryepiglottic fold; it encompassed the arytenoid cartilage and false vocal fold, and it extended into the paraglottic space.
Contrast-enhanced computed tomography (CT) of the neck demonstrated a homogeneous soft-tissue lesion that involved the left paraglottic space and glottis and extended into the subglottis (figure 1).
The mass extended to the right false vocal fold, paraglottic space, and subglottis.
The mass had obliterated the paraglottic space and distorted the thyroid cartilage.
This is precisely the expected location of the lesion as the inflammatory mass expanded and followed the path of least resistance inferiorly through the loose tissue of the paraglottic space.
The hemorrhage in this case resulted from manipulation within the paraglottic space, perhaps from trauma to the inner perichondrium of the thyroid cartilage.
Axial computed tomography (CT) showed a heterogeneously enhancing soft-tissue mass involving both the false and truevocal folds and extending to the supraglottis, arytenoids, and paraglottic space (figure 1).

Injection laryngoplasty does pose a risk of postprocedure bleeding into the paraglottic space and occasionally into Reinke's space, and this can lead to prolonged dysphonia.
However, conservation procedures should be performed whenever possible, as we did for our patient, because these tumors permeate focally rather than through the paraglottic space.
Computed tomography (CT) of the larynx revealed the presence of Teflon in the lateral aspect of the left paraglottic space (figure 2).
Specifically, tumor progression occurs primarily within the paraglottic space and extralaryngeal compartments; the potential for mucosal spread is limited.
