Fischer syndrome Neurology A unilateral pontine lesion involving the medial longitudinal fasciculus and the pontine paramedian reticular formation, resulting in ipsilateral gaze palsy and internuclear ophthalmoplegia on contralateral gaze; the only remaining horizontal movement is abduction of the contralateral eye; the eyes are straight or exodeviated Etiology Focal lesions of brain stem–eg, multiple sclerosis, 1º or 2º tumors–eg, glioma, hemorrhage or infarction, AV malformations, basilar artery aneurysm
