disruption of a ligament, which may be partial or complete. The resultant bleeding causes pain, swelling and loss of function. Ligament tears are graded 1-3 in increasing severity: (1) microtears and stretching but ligament integrity intact; (2) partial disruption with laxity but a discernible endpoint on stressing; (3) complete rupture. Grade 3 tears, though most severe, may be relatively painless if nerve fibres are also torn. Severe or complete tears disrupt joint function and stability. Sometimes a bony fragment is torn off the bone at the ligament attachment - known as an avulsion fracture. Treatment includes RICE and anti-inflammatory medication (though 'Rest', i.e. complete immobilization, is now rare, to avoid secondary muscle wasting and loss of function). Surgery may be indicated if return to sport is important, e.g. after complete anterior cruciate ligament rupture. A graduated rehabilitation programme is required with a gradual and progressive increase in strength work, balance and flexibility, before returning to sport or exercise.
