The cupulolithiasis theory holds that BPPV is caused by the presence of otoconial debris that adheres to the cupula of the posterior semicircular canal.
In the presence of gravity, this temperature gradient results in the convective flow of endolymph that deflects the cupula and generates nystagmus.
Rotation of the head causes a flow of fluid, which in turn causes displacement of the top portion of the hair cells that are embedded in the jelly-like cupula.
