This incomplete relaxation of the bridge during diastole results in increased intracoronary flow velocities, reduced diastolic coronary flow, retrograde coronary flow, and a reduction in coronary flow reserve, resulting in a lowered ischemic threshold (6).
Serruys reported that the preliminary six-month data from the DEBATE study showed that patients with low coronary flow reserve, as measured by Cardiometrics' FloWire(R) Doppler guide wire, immediately following balloon angioplasty were approximately 50% more likely than patients with high coronary flow reserve to suffer a major adverse clinical event (mainly repeat angioplasty procedures) within six months of their initial angioplasty.
But in this randomized study, we are comparing two practice patterns -- one in which all lesions are stented and the other in which stents are implanted only when coronary flow reserve (CFR) is low or the angiogram reveals a relatively high residual blockage after angioplasty.
Authors have clearly shown that a myocardial bridge in this patient caused myocardial infarction possibly by impaired coronary flow reserve and endothelial dysfunction.
