blue chip

A small flat plastic chit with embossed lettering which contains basic patient demographics, including name, medical record number, date of birth, date of admission, physician of record, type of insurance, room number, etc., which is “swiped” in a credit card-like fashion on orders for lab work, imaging, billing, and other inpatient procedures.
