| Our guideline criteria do not differentiate autologous from allogeneic transfusions because of the error and risks associated with autologous transfusion. The Blood Bank of San Bernardino and Riverside Counties, San Bernardino, CA, was not cited for any 483 violations following a 2003 inspection, but did receive a one-item 483 in 2005 because SOPs for the collection and processing of blood and blood components for homologous transfusion and autologous transfusion were not always followed. Moreover, records were not concurrently maintained with the performance of each significant step in the collection and processing of each unit of blood and blood components; and the facility failed to maintain written SOPs, including all steps to be followed in the collection, processing, storage and distribution of blood and blood components for homologous transfusion, autologous transfusion and further manufacturing purposes. |
