patient health record identifier

A 2-part alphanumeric code which allows an authorised user to access a patient's clinical notes within a UK hospital or group of hospitals. Part 1 identifies the health records system and Part 2 identifies the patient within the records system. Together, the 2 parts flag the patient’s health records (case records) and, when paired with the hospital location code, enable unique access to a patient’s health record country-wide.
