(15) The patient gradually responded to standard MB-MDT as she had enlargement of two
nerve trunks (without clinical evidence of neuritis) in addition to the skin lesion.
In his second case, Scaramella used the buccal branch for an anastomosis with a sural nerve graft to the paralyzed facial
nerve trunk. (4)
These tumours may affect any nerve in the body mainly in the flexor surfaces of limb and
nerve trunks, often at origin of spinal or cranial nerves.1 They usually grow slowly and appear as painless swellings for several years before being diagnosed.
The facial
nerve trunk was then carefully located by blunt dissection as it emerges from the stylomastoid foramen immediately above the attachment of the posterior belly of digastric muscle and along the anterior border of the mastoid process.
(1) Pure or primary neuritic leprosy (PNL) only involves peripheral
nerve trunks without skin lesions.
The main facial
nerve trunk had passed through the mass.
Kilik et al (2010) found double facial
nerve trunk. Katzand Catalano (1987) found two main trunks, major and minor trunk in 3% of cases.
Neuropathic pain is a common complaint due to leprosy neuritis, whether spontaneous or on palpation of a
nerve trunk. It can be present during or after treatment with multidrug therapy, however the chronic neuropathic pain in treated leprosy is arising as a major problem.
Neuritis (N)--Patients presenting with acute inflammation of one or more peripheral
nerve trunk detectable by swelling and/or functional impairment with spontaneous nerve pain and/or nerve tenderness on palpation.
In a truncal vagotomy, the
nerve trunk is fully resected.
This would help to dissect and identify facial
nerve trunk at the inferior edge and facial nerve branches at the superior edge.
As for electrode placement, Bergquist, Clair, and Collins (2011) argued for placement of the electrode over the
nerve trunk rather than the muscle belly in order to promote a greater central contribution to motor unit recruitment (Bergquist, Clair, & Collins, 2011).
