Community care patient/client:
• Has an identifiable physical condition or mental illness or disability.
• Is an expectant or nursing mother receiving community care services
• Has contact with community care professionals as part of a population screening, preventative medicine or health promotion programme.
• Has a condition (e.g., frailty) requiring community healthcare.
