a synthetic analogue of calcitriol, to which it is converted in the liver; used in the treatment of hypocalcemia, hypophosphatemia, rickets, and osteodystrophy associated with various medical conditions including chronic renal failure and hypoparathyroidism, administered orally or intravenously.

alfacalcidol

A synthetic form of vitamin D used to treat low blood calcium and bone softening (OSTEOMALACIA) caused by kidney disease. Brand names are Alfad and One-alpha.

Generally, a good response to treatment with alfacalcidol or calcitriol (10-400 ng/kg/day) is expected in cases with VDDR1A (4,21).
Most clinicians (84%, n=158) prescribe Cholecalciferol, 19 (10.1%) Alfacalcidol and 11 (5.9%) prescribe Calcitriol as a vitamin D supplement, while 155 (82.4%) prescribe a maintenance dose.
As expected, most studies compared bisphosphonates with Vitamin D analogs (cholecalciferol, alfacalcidol, and calcitriol) or placebo treatment.
The patient was placed on active vitamin D (alfacalcidol) and calcium supplements.

This patient was initially treated with an intravenous calcium infusion and subsequently started on chronic oral calcium supplementation along with alfacalcidol to maintain a serum calcium level in the low normal range.
For the management of CKD-MBD, 76.9% of the patients were on calcium carbonate and 64.3% on alfacalcidol with a similar distribution of drug usage across the groups.
