Pipelle

A proprietary name for a device used to obtain a sample of endometrium without DILATATION AND CURETTAGE. It consists of a flexible polypropylene sheath with a soft, rounded end, containing an inner piston. The blunt end, which has a perforation near the tip, is passed through the cervix into the womb and the piston drawn sharply back. In this way a small core sample of tissue is obtained. The end of the sheath is then cut off and sent to a laboratory for histological examination. The procedure can often be performed without anaesthesia.

Endometrial samples were obtained by dilatation and curettage or pipelle biopsy.
Endometrial sampling was performed using a Pipelle device, and dilatation and curettage was required for two patients due to cervical stenosis.
Samples were collected by way of various methods, including Pap smear, endocervical brushings and curettage, and endometrial pipelle. The pathologic findings in endocervical samples coincided with the final surgical pathology in 9 of 10 patients with benign findings, 1 of 6 patients with dysplasia, and 2 of 2 patients with cancer.
Further, a prehysterectomy study by Rodriguez et al demonstrated that Pipelle endometrial biopsy sampled an average of only 4% (range, 0%-12%) of the endometrial surface area.
Methods: This double-blind, placebo-controlled trial was conducted at Fatih Sultan Mehmet Training and Research Hospital, Istanbul, Turkey, from May to November 2015, and comprised patients undergoing endometrial biopsy with Pipelle. Intrauterine lidocaine infusion, paracervical block with lidocaine, intravenous tenoxicam or 4ml intravenous normal saline administered prior to biopsy.

NPs are able to make a diagnosis, recommend and prescribe treatment and carry out procedures such as pipelle (endometrial) biopsy and Mirena insertion, if that is the treatment option the woman chooses.
Infertile patients with good response to controlled ovarian hyper-stimulation (COH) but with one or two unsuccessful IVF/ET attempts were treated at least 4 times by endometrial biopsy using a biopsy catheter (pipelle device) during the menstrual cycle, before the next IVF-ET cycle.
