Med 3

A now-retired sick note form for UK workers that was filled out by a patient’s GP free of charge, which certified that the patient was unfit for work. It was replaced in 2010 with the Statement of Fitness for Work.

There was a contractual obligation for any GP who issued a Med 3 to provide medical reports in relation to Incapacity Benefit and Employment and Support Allowance.
