Two small incisions are made laterally and below the anal margin to provide an entry to the fossa, and the tunneler is used to pierce the iliococcygeus muscle below the sacrospinous ligament to reach the vaginal incision.
The patterns of levator ani morphology were based on the width of the iliococcygeus muscle and the iliococcygeal angle, which measured the angle of "take-off" of the ileococcygeus in comparison with an imaginary line drawn between the ischial spines on either side.
The levator ani is further made up of the pubococcygeus, puborectalis, and iliococcygeus muscles. The urethral and anal sphincter muscles are also part of the pelvic floor.
