A fulminant subcutaneous bacterial infection of the genital and anorectal regions, characterized by necrosis–cellulitis, fasciitis, myositis rapid progression, severe systemic toxicity, lack of suppuration; most Pts are ♂, ± age 50 Etiology Periurethritis with urine extravastion, postsurgery, postinstrumentation, septic injection in dorsal vein of penis Underlying diseases DM, IVDA, CA Clinical Systemic toxicity, urinary retention, abdominal discomfort, regional necrosis, fever, leukocytosis Treatment Debridement, broad-spectrum antibiotics Controversial therapies High-dose prednisone, hyperbaric oxygen
