B-cell lymphoproliferative syndrome

An uncommon, life-threatening complication of bone marrow (BM) or organ transplantation caused by profound immunosuppression, which may be induced by EBV. BLS occurs in < 0.5% of the recipients of HLA-identical BM, but is common in patients who suffered severe GVHD and were treated with anti-CD3 antibodies.
 
Clinical findings
Ranges from self-limited, spontaneously resolving infectious mononucleosis to oligoclonal or monoclonal proliferations and aggressive lymphomas.
 
Prognosis
80–90% mortality in BM recipients; 60% survival in those receiving other organs.
