This Phase III study (NCT02195479) is a randomized, open-label, multicenter study that included 706 newly diagnosed patients with multiple myeloma who are ineligible for
ASCT. Patients were randomized to receive 9 cycles of either VMP [bortezomib (a proteasome inhibitor), melphalan (an alkylating chemotherapeutic agent) and prednisone (a corticosteroid)] combined with daratumumab, or VMP alone.
A large international Phase 2 study of pidilizumab conducted on 66 patients with measurable disease after
ASCT showed good outcomes with 51% ORR (34% CR, n=12; PR 17%, n=6) (30).
In a retrospective study at IU [7], 184 consecutive patients with testicular or retroperitoneal primary GCT underwent two cycles of high-dose carboplatin and etoposide, each followed by
ASCT. Carboplatin dose was calculated using body surface area (BSA), and patients in remission after HDCT received adjuvant oral etoposide for 3 months.
After detailed review, a total of 43 studies were further excluded for the following reasons: duplicate study data, not focused on relapsed sarcoma, or not receiving
ASCT. A total of 21 articles met our inclusion criteria.
However, there are reports of disseminated disease [7], retinochoroiditis [8], or solitary spinal cord lesion [9] following
ASCT. Cerebral toxoplasmosis has been described in one case each following
ASCT for non-Hodgkin's lymphoma [10], neuroblastoma in a pediatric patient [11], and after tandem
ASCT for myeloma [12].
Post
ASCT, invasive fungal infection [IFI] usually occurs in pre-engraftment period however can happen after count recovery as well.
