A thrill may be felt at the lower left sternal border (VSD), left upper sternal border (pulmonary valve stenosis) or suprasternal notch (aortic stenosis).
But if you know the anatomy there's a way to do it: The arrow must hit right on top of the suprasternal notch, the dip at the top of the breastbone just below the Adam's apple.
