Polycystic ovaries, sclerocystic ovary disease, Stein-Leventhal syndrome Gynecology An idiopathic condition affecting 3.5-7.0% of ♀, and most common cause of familial hirsutism Clinical Obesity, hirsutism, galactorrhea, 2º amenorrhea following dysmenorrhea, acne vulgaris, and an ↑ risk of endometrial carcinoma due to unopposed estrogenic stimulation; PCO may be associated with CNS trauma or injury in childhood Diagnosis Palpation, ultrasonography Lab ↑ LH or LH/FSH ratio, ↑ prolactin, ↑ response to nafarelin–a GnRH-gonadotropin-releasing hormone agonist that causes a ♂ pattern response, suggesting that PCO has defective regulation of 17-hydroxylase and C-17,20-lyase; sterility, menstrual abnormalities and hyperandrogenism ± associated with valproate therapy for epilepsy.
