When Mrs Hopkins retired in 2006 she received a letter from Sir William Wells, chairman of the NHS appointments commission, on behalf of the Secretary of State for Health, praising her role in developing PCTs after their creation in 2002.
The appointments of Brian Ebbatson and Malcolm Crook have been made by the NHS Appointments Commission, in accordance with the Office of the Commissioner for Public Appointments' code of practice.
The authority asked the NHS Appointments Commission to consider the position of James Lee, the trust's chairman, who was reappointed to a second four-year term in June.
