1 major and 2 minor criteria, assuming previous evidence of group A streptococcal infection

• Eythema marginatum
• Subcutaneous nodules

Clinical findings
• Arthralgia
• Fever
Laboratory findings
• ↑ Acute phase reactants
• Erythrocyte sedimentation rate
• C-reactive protein
• Prolonged PR interval
Evidence of previous group A streptococcal infection
+ Throat culture or rapid streptococcal antigen test
↑ (or rising) streptococcal antibody titer
