Diagnosis is based on radiographic findings of bilateral hilar lymphadenopathy, with or without parenchymal infiltrates, and is supported by the presence of non-caseating granulomas on pathologic evaluation. Radiographic abnormalities associated with coccidioidal infection included pulmonary infiltrates in 88% and hilar adenopathy in 1 participant. Computed tomography of the patient's chest and abdomen confirmed a right hilar mass completely obstructing the right middle-lobe bronchus with distal pneumonitis and large mediastinal nodes. |
