echinocandins

echinocandins

A class of lipopeptide antifungal drugs that act by inhibiting glucan synthesis, thereby damaging fungal cell walls. They are effective against Candida and Aspergillus species. Examples are caspofungin (Cancidas) and micafungin.

Most filamentous Basidiomycetes are susceptible to antifungal drugs except for fluconazole and echinocandins.
However, there have been cases of development of resistance to echinocandins while on therapy.
Echinocandins such as caspofungin and micafungin have shown considerable in vitro and in vivo efficacy in the treatment of invasive candidiasis and candidemia.
The fungal strain is typically multidrug resistant to all major classes of antifungals, almost always to polyenes and azoles, and in many cases to echinocandins (CDC, 2017c).
After incubation, the concentration that enabled 100% inhibition for AMB, 80% inhibation for azoles and echinocandins, and 90% inhibition for FLU was specified as the minimum inhibitory concentration (MIC) (8).
20 These species were normally found susceptible to amphotericin B and were resistant to triazoles and Echinocandins,21 which also supported our findings where isolated strains of Mucor spp.
Antifungal susceptibility testing at CDC of the first 35 clinical isolates revealed that 30 (86%) isolates were resistant to fluconazole (minimum inhibitory concentration [MIC] >32), 15 (43%) were resistant to amphotericin B (MIC [greater than or equal to] 2), and one (3%) was resistant to echinocandins (MIC > 4).
19,20) Resistance of Candida species, such as Candida glabrata, to both fluconazole and echinocandins is a real healthcare concern.
For example, the echinocandins are fungicidal and exhibit long PAFE against Candida spp.

The major drug classes for treatment include azoles, polyenes and echinocandins.
