Form 14 Coroner’s Statement
A form issued only if the Coroner has decided that neither an inquest nor a post-mortem examination is necessary.

Form 17 Coroner’s Certificate
A form issued only if the Coroner has decided that a post-mortem examination, but not an inquest, is needed.

Form 21 Coroner’s Certificate
A form issued when the Coroner deems it necessary to hold an inquest into the death.
