community mental health team

A UK term for a multidisciplinary team of health and social services professionals (psychiatrists, social workers, community psychiatric nurses, psychologists and mental health therapists), which offers specialist mental health assessment, treatment and care to people with severe and/or long-term mental health problems in their home or community, rather than in hospital.
